As a key member of our Carolina Health Centers, Inc. team, the Physician will play a vital role in establishing and overseeing a comprehensive, community-based primary care program. The primary objective of this role is to enhance access to high-quality primary care services, focusing on family-centered care. The Physician will be responsible for designing and implementing an integrated delivery model that prioritizes prevention, health maintenance, health education, and evidence-based chronic disease management. By collaborating with a multidisciplinary team, the Physician will contribute to improving health outcomes and promoting wellness within the community.

The ideal candidate will have a deep commitment to delivering patient-centered care and building strong relationships with patients and their families. Strong communication and interpersonal skills are essential for effective collaboration with colleagues and engaging with diverse patient populations. The Physician should be well-versed in current best practices in primary care, preventive medicine, and chronic disease management.

Qualifications

Education and Experience
• Graduate of an accredited Medical School
• Completion of a family medicine residency in an accredited program

Licensure and Certification
• Active South Carolina medical license
• Board certified or Board eligible in Family Medicine/Pediatrics

Working Hours

30 hours min or Full-time
