InnovAge is a leader in providing comprehensive healthcare services to geriatric participants through our innovative PACE (Program of All-Inclusive Care for the Elderly) model. We are dedicated to enhancing the quality of life for our enrolled participants by delivering personalized medical care in various settings.

Responsibilities:
- Provide medical care to enrolled geriatric participants across multiple settings including Day Center clinics, participants' homes, nursing homes, assisted living facilities, and personal care boarding homes.
- Seeing 8-10 patients a day (patient panel 75 - 100)
- Perform evaluations, diagnosis, and management of medical conditions, including medical history review, physical examinations, diagnostic test interpretation, treatment planning, prescription writing, and ongoing follow-up.
- Document medical care accurately in participant's electronic medical records, adhering to InnovAge PACE documentation policies.
- Participate in interdisciplinary team meetings to formulate comprehensive care plans for participants.
- Contribute to quality improvement initiatives and assist in the establishment and enhancement of electronic medical records.
- Provide on-call coverage on a rotational basis, supporting after-hours medical advice and orders as needed (phone support only).
- Present input into and facilitate new medical programs and processes to support strategic goals.
- Lead by example to maintain a positive department culture, fostering morale, teamwork, and organizational values.

Requirements:
- Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) degree from an accredited medical school.
- Current active state Medical License and DEA license with full prescriptive authority.
- Board Certification in a primary care specialty, or Board Eligible with certification required within the first 12 months of employment.
- Minimum of one year of experience working with frail or elderly populations.
- Not on the CMS Preclusion List.

Preferred Qualifications:
- Postgraduate training in a primary care specialty such as internal medicine or family medicine.
- Completion of post-residency training in geriatric medicine.
- Bilingual proficiency in English and [specific language].
