Position Overview:

The Physician's overall responsibility is to provide a full scope of primary care services which fall under their field of training. The Physician is to administer coordinated and comprehensive professional health care services that include, but are not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance for the patients at SMF. In addition, the physician is part of a Patient Centered Medial Home (PCMH) team and is expected to be familiar with the philosophy 1 goals and objectives of the PCMH.

Job Responsibilities

Job Duties:

Assess, diagnose, implement treatment, evaluate and monitor patient's health status (PCMH).
Examines patient, performs comprehensive physical examination (PCMH).
Obtains complete medical history from patient, family and previous medical records if indicated (PCMH).
Order routine and diagnostic lab and radiological studies and other necessary tests as indicated and interpret results (PCMH).
Collaborates with the health team members to establish priorities, manage care and coordinate efforts to achieve desired outcomes (PCMH).
Coordinates care with consulting physicians. community resources, hospital, and ER or Urgent Care (PCMH).
Participate in pre-visit huddle to ensure smoother workflow (PCMH}.
Assess barriers to meeting treatment goals and advise patient care team of impacts to plan of care (PCMH}.
Work collaborative with other members of health care team (PCMH).
Ensure care coordination, referral management, clinical workflow processes by meeting regularly with clinical staff (PCMH).
Provide clinical care using team-based approach centered on patient needs and incorporating individual patient preferences in the delivery and approach to care (PCMH).
Participate in peer reviews, quality assurance, provider meeting, and other clinical meeting as needed.
Maintains confidentiality and privacy in accordance with HIPPA and SMF.
Remain current with the trends and professional development.
Exhibit professional conduct at all times.
Maintain accurate and timely charting of patient encounters.

License/Certification:
DEA Certification (Preferred)

Skills

Knowledge/Skill Requirements:

Obtain and maintain current South Carolina Medical License.
Be Board Eligible or Board Certified as an MD/ or DO from an accredited medical school.
Maintain Continuing Medical Education credits as required by South Carolina Medical Board.
Current Cardiopulmonary Resuscitation certificate (CPR)ACLS or Advance Life Support within 12 months of employment, if appropriate.
Proven experience as a qualified primary care physician and satisfactorily completed residency training in family or general practice of medicine or completed rotating internship.
Ability to work cooperatively with patients and health care staff members in performance of assigned duties.
