Sandhills Medical Foundation, Inc. has been providing quality comprehensive health care since 1977. It is a Federally qualified community health center (FQHC) serving residents of Chesterfield, Kershaw, Lancaster, and Sumter Counties.

According to the Health Resources Services Administration, health centers are community-based and patient-directed organizations that deliver comprehensive, culturally competent, high-quality primary health care services to the nation’s most vulnerable individuals and families, including people experiencing homelessness, agricultural workers, residents of public housing, and veterans. As a patient centered medical home provider, Sandhills Medical Foundation provides comprehensive healthcare to all individuals. We provide services such as primary care, mental health care, chiropractic, onsite pharmacy and lab services, to meet the needs of our patients. Sandhills also provide healthcare services in the community such as patient education, flu shots, and health screenings such as blood pressure, diabetes, and cholesterol and HIV.
