Outpatient Family Medicine Openings with FQHC in Northwest Indiana just a short drive from downtown Chicago
- Outpatient only
- No call
- My client is a very rapidly growing FQHC in Northwest Indiana.
- They prided themselves on being a flexible place of employment with emphasis on work/life balance.
- They have schedules such as 5-8s, 4-10s, some longer days, some shorter, whatever works best for the provider, within reason.
- Since they are a FQHC, they also can offer the HRSA loan repayment option for providers
- Credentialing takes up to 3 months, and once the provider starts, they allow the provider to shadow multiple providers throughout their clinics and specialties to learn their culture, workflow, EHR (NextGen), etc.
Compensation/Benefits
- Salary: $230,000-$250,000 plus productivity/quality quarterly bonus
- Vacation: 160 hours
- Personal time: 40 hours
- CME: 40 hours + allowance of $2500
- Medical, dental and vision insurance, life insurance that is 1.5x the provider’s salary, short-term and long-term disability, a 401k plan that the client matches up to 4%, FTCA malpractice insurance
Client Details:
- They are a non-profit organization dedicated to providing quality, affordable healthcare to everyone, including the uninsured and underinsured in Porter, Lake, La Porte and Jasper Counties.
- They currently have over 20 locations between full service clinics and primary care clinics in local school districts and Strack & Van Til grocery stores that provide medical, dental, vision, chiropractic, mental health and medication assistance through onsite pharmacies. Patients are seen on both a scheduled and walk-in basis.

- All of their FQHC’s are located in northwestern Indiana anywhere from about 20 – 45 miles of downtown Chicago
