Providers provide routine primary care, wellness assessments and refer for specialty care
Join a location with 5 other providers and a pharmacy onsite
Provide patient care to patients of all ages (pediatric, adolescent, women's health and adults)
Monday-Friday schedule, 9:30am-5:30 pm with NO WEEKENDS
Evaluate patients medication needs, write prescriptions
Communicate with case workers, RN teams and other ancillary staff to ensure treatment plans are carried out
Provide patient education around health promotion, disease prevention, and health maintenance
