Facility in Durango, CO seeking Urology locums coverage

Rates: $350/hr clinic. $3000 per call with 4 hour gratis. $800/night call with 2.5 hours gratis. $3000/24 hour weekend call with 4 hour gratis. $375/hr call back

Dates of Coverage:

• Nov 11 (8a) - 18 (7a)

• 1 week per month ongoing starting January 2025.

Schedule/Availability:

• Clinic & Call Coverage:

  • Monday-Friday: 8a-4p + 4p-7a (night call).

  • Saturday-Sunday: 24-hour call (7a-7a).

  • Setting: Inpatient, Outpatient, Emergency Room, Clinic, Trauma Level III.

  • FTE: 0.5.

  • EMR: EPIC.

Details:

• Support Staff: MA, NP/PA, RN.

• Procedures/Job Duties:

  • Seeing patients in the clinic.

  • OR time for various procedures.

• Reason for Coverage: Backfill needed due to provider cancellation.

Credentialing Information:

• Timeframe: 60-90 days.

• Emergency Temp Privileges: Available, less than 30 days for clean files.

• EMR Training: Virtual and onsite training scheduled for the provider’s first day.

• Credentialing with Other Facilities: No.

Requirements:

• Board Certified Urologist - Required, no BE candidates.

• Active Colorado License or IMLC (LOQ required at submission).

• Active BLS, DEA - Required for credentialing (transferred DEA acceptable, but must retain for duration of assignment).

• Availability - Must be available for 80% of Sept-Dec 2024 dates to be considered.

• Must provide number of shifts available per month ongoing.

• Malpractice: Site willing to accept 1 case up to $300k, must provide malpractice explanation.

• Clean Background - Required for urgent shifts.
