Setting: Free Standing Clinic
Dates of coverage needed: As soon as credentialed - 10/31 with possible ongoing extension
Schedule: Monday – Friday; 7a - 4p
Scope of Practice: Urgent Care, work related injuries, pre-employment physicals
Experience: 2+ years of UC and Occ Med experience
License and certification: Board Certified, KY State License, DEA, BLS, DOT Certification preferred
