Requirements:

• Board Certified Pediatric Surgeon – REQUIRED
• VA State License & DEA – REQUIRED at time of submission
• CLEAN BACKGROUND – NO EXCEPTIONS
• COI coverage of $2.6M/$7.8M (VA statute limitations) – Required if requested by the facility
• MUST BE AVAILABLE FOR THE LISTED DATES
