Qualifications
REQUIRED QUALIFICATIONS: Education: Doctorate DDS or DMD Licenses/Certifications: DENTST-Dentist Other Licenses/Certifications: CODA Accredited General Practice Resindency (GPR) certificate OR Certificate of Post-graduate Fellowship OR Military Service Current State of Illinois Dental License, Controlled Substance DEA License Experience: 5 years of General Dentistry experience Other skills/knowledge: Excellent interpersonal and communication skills.
