• Board Certified Ortho Surg – Required (no BE)
• MI license and MI CSR at time of submission – Required (not accepting IMLC)
• Rates and exact availability must be submitted at time of name clear – Required will not send to client without rates
• BLS
• ACLS – Required
• Clean Background – Required for emergency privileging
