- October 15th @ 7am - October 24th @ 7am
- November 7th @ 7am - November 8th @ 9pm
- November 15th @ 7am - November 21st @ 7am
- December 5th @ 7am - December 6th @ 9pm
- December 13th @ 7am - Dec 19th @ 7am
- 24 hour call from 7a to 7a up to 15 days per month
- Clinical Information:
- Practice EMR: Meditech
- Support Staff: Hospital and OR staff
- Required:
- DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
- Current Flu Vaccine
- Must be fully vaccinated for Covid 19
- Exemptions only allowed for religious or medical reasons, once approved by the facility.
