Practice Setting: Office-Clinic w/ hospital coverage

Schedule: 8am - 5pm

Average Number of Cases Per Day: 1 hospital consult every 1-2 weeks

Average Number of Patients per Day: 12

Documentation System/EMR: Meditech Expanse

Number of Exam Rooms: 7

Number of Rheumatologist in Practice: 2 MDs & 1 PA

Description of Support Staff: 4 MA's, 1 LPN, 2 receptionists

Patient Age Demographic: (Adolescent, Adult, Geriatric): Adult

What Percentage of Practice is Arthritis, Autoimmune, Chronic Fatigue, Internal Medicine?: 100%

Is Board Certification Required - Yes - ABIM, ACR

Do You Require Fellowship Training?: No

Will you Accept Providers Needing to Obtain State License? Yes

Certification Requirement: BLS, ACLS

Estimated Credentialing Timeframe: 3 months
