• Board Certified
• State License – ND License is preferred – IMLC is okay
• Residency Requirements/Fellowship – REQUIRED
• ACLS – REQUIRED
• Malpractice – Clean Preferred
• Availability at time of name clear – REQUIRED
• Must have experience with all required procedures
