Job Responsibilities
Qualifications
Radiologist experience required, including ability to read CT, MRI, Ultrasound andDiagnostic X-ray Imaging within the last (10) years.
Evidence of Fellowship training in either Neuroradiology, Musculoskeletal (MSK), or Abdominal/GU, (Body) imaging is required.
Evidence of professional licensure/certification and transcript or equivalent from educational institution(s) certifying completion of graduation requirements. All contractor's physician(s) shall be Board Certified per the American Board of Radiology (http://www.theabr.org/). All continuing education courses required for maintaining certification must be kept up to date at all times.
Evidence of Board Certification by the American Board of Radiology (ABR) in either Abdominal/GU, Breast Imaging, Cardiovascular, General, Musculoskeletal, Neuroradiology Services with a fellowship in either Body imaging, Musculoskeletal (MSK) or Neuroradiology.
Copies of all licenses, board certifications, NPI, to include primary source verification of all licensed and certified staff. Licenses must be current, full, and unrestricted.
Certification that staff list has been compared to OIG list
Proof of Indemnification and Medical Liability Insurance
Prior malpractice claims for each provider must be submitted.
