• Board Certified/Board Eligible
• CA Active license
• Availability at time of name clear
• Residency (Ability to perform diagnostic mammograms and biopsies)
• Clean Malpractice (Highly Preferred)
• MQSA Certification
• BLS/ACLS and DEA (Required during credentialing
