Pay rate:
$325/hr
Schedule/Availability Requirements:
- clinic M-F 0800-1700
- Every week M-F
- 9/9/24-2/9/25
- **Closed 11/28-11/29; 12/25; 1/1/25; 1/20/25
- Setting: Clinic and consults if a patient they are seeing is admitted
- FTE: 1.0
- Duties: New Patient Consults
- Required Procedures: N/A
- EMR: Cerner
- Support Staff: Medical Assistant, Front Office Assistant
- Reason for Coverage: Physician LOA
Additional Details / Questions Answered by Facility:
- Approximately 10-12 new patients a day
Credentialing Information:
- Timeframe: 1-2 months
- Requirements: MD, DO
- Credentialing with other facilities?
- Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less?: Yes
REQUIREMENTS
- Board Certified in Pulmonology - REQUIRED
- AZ License or IMLC (LOQ must be in hand) - REQUIRED
- DEA - REQUIRED
- Clean Malpractice/background - PREFERRED
