New locum opened for PM&R physician in Santa Rosa, CA
Open to CA license only
April 28th start and Ongoing need
Looking for 2 provider's total
No call
Inpatient Coverage
10-15 ppd
BE/BC open to apply
Level 1 facility
30-60 day credentialing
CA license accepted only
LOCUM- Physical Medicine & Rehabilitation
We are looking for two providers for this need. Ideally, we would like to privilege them at QVH & PVH for locum needs as they arise there as well.
The site is working on obtaining temporary privilege approval for the April shifts. If this can not be obtained provider will more than likely not start until June.
OT Policy: Overtime must be pre-approved by medical director prior to end of shift
Presents should reflect the following requirements:
Guaranteed Hours: 8
OT applies after: 10 hours worked
Providers must take 1 hour lunch
Specific Dates Needed (Provider must have availability or will not be considered):
Apr 28-May 18
Jun 9-Jun 18
Sep 29-Oct 5
Nov 24-Nov 30
Dec 22-Jan 4
