Physical Medicine & Rehabilitation Locum position in Santa Rosa, CA: $175 Hourly

Start Date: April/June - Ongoing ((The site is working on obtaining temporary privilege approval for the April shifts. If this can not be obtained, the provider will more than likely not start until June.)

Coverage Type: Scheduled Clinical Hours Only

Submission Requirements:

1. Active CA License Only: REQUIRED
2. Active Board Certification: REQUIRED
3. Active ACLS, BLS Certifications: REQUIRED
4. Availability for all dates needed: REQUIRED

-Define The Number of Support Staff to Be Supplied For The Locum Coverage: RN: 2-4, MA: n/a, PA/NPs: 1, Office Staff: 3
-Will Provider Be Required To Supervise Advance Practice Providers?
-Resident or Teaching Supervision Required? No PA/NPs: 1 Office Staff: 3
-EMR System: Epic
-Will Facility Accept Multiple Providers? Yes
-Is Hospital Stroke Certified? Yes
-Patients Per Shift: 10-15
