*APC's with 3 years of experience in a similar setting

*PAs must have a CA Controlled Substance Education Course Certificate

*NPs must have a Furnishing License with Schedules II

*Outpatient
*No call

*Active DEA needed before start

*The following immunizations are required for credentialing (before the start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within 90 days before the start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years), COVID requirement:
