Candidate Requirements
Active IN State License Board Status: Board Certified or Board Eligible (Certification must be obtained within 5 years of residency). Minimum Years Experience (if applicable): More than 2 years Flu vaccination: Required or Need Medical or religious exemption. Certifications: BLS-AHA issued.
