We are in search of an ENT Physician for Northern Indiana. The region is located 30 minutes away from Chicago. (Chicagoland)

Location: Chicagoland, Indiana.

Job Type: Full time / Direct Hire / Staff

Shifts: Days, M-F

Practice Details:

• Full-time/permanent position with inpatient and outpatient practice
• Unique work structure conducive to utilizing all your skills
• Currently 3 ENT's in the group
• Will take call at more than one facility 1:4
• Outpatient Surgery
• EMR: EPIC - fully integrated HIMSS Stage VII (Only 5% of hospitals in country are at this level of sophistication)
• Easy to use; support department available

Requirements:

• Indiana license
• Board Certified / Truly Board-eligible. (Within 5 years of residency.)

Stage 1 - "Name-clear" - Have they heard of you or worked with you before?

• One intro phone call to go over job details, field questions and learn more about you.
• Current Up-to-Date CV (including Locums history)
• Provider's Contact Info
• Availability (For interview & 2025 availability on-going.)
• NPI and/or License Number.

Stage 2 - "Full-Facility Submission" - Introducing you to the Leadership at the facility.

• All pertinent Licenses & Certifications
• Malpractice and/or Background Documentation (NPD)
• References if Available.
• Scope of Services if Applicable
