Practice Setting: Hospital-Based
Location : Southern Tier of New York
Certification Requirements: Board Certified
State License Requirements: New York License Preferred
Additional License Requirements: BLS, ACLS
Number of RAD Onc Cases Per Day : 18
Patient Population: Adult
Required Procedures/Responsibilities:

• Conventional Radiotherapy, 3D Radiotherapy, SBRT, External Beam Radiation - 3D-CRT, External Beam Radiation - IGRT, External Beam Radiation - IMRT, Stereotactic Body Radiation Therapy-SBRT
• Required Duties: Orders Tests, Prescribes, Orders Images

Average Time of Credentialing: 60-90 days
Dates of Coverage: 12/23-12/27 , 12/30-1/3/2025 , Ongoing needs into 2025
