• Average Number of Patients Per Day - OUTPATIENT (Clinic/Office) : 25
• Average Number of Patients Per Day - INPATIENT (Call/Rounding) : variable 1 per week
• Patient Population for Required Coverage: Adult, Office/Clinic, Hospital Based
• Documentation System/EMR: meditech/ mosaiq
• Estimated Credentialing Timeframe: 60 days approx.
• Provider Must be local - only mileage is reimbursed

• 3D Radiotherapy
• 3D conformal, IMRT, SBRT and SRS (No brachy needed)
• External Beam Radiation - 3D-CRT
• External Beam Radiation - IGRT

• Stereotactic Radiosurgery
• Stereotactic Body Radiation Therapy (SBRT)
• Fractionated Stereotactic Radiotherapy
