SUBMISSION REQUIREMENTS
Board Certified - REQUIRED
OH License - Preferred; willing to accept providers needing to obtain license
BLS/ACLS Certified - REQUIRED
FULL TIME AVAILABILTY ONLY
Flu vaccine - REQUIRED at time of credentialing
COVID Vaccine - Preferred
