Services Provided:
Provide expert consultation and treatment to patients in an inpatient and outpatient clinic setting. The physician will evaluate, triage and prioritize consults from primary care and other services for outpatient oncology consultations. The physician will obtain medical history, conduct physical examinations, order and evaluate appropriate laboratory testing, and plan and prescribe treatment for patients. The physician will coordinate medical management and care with Primary Care providers and other personnel in a multidisciplinary health care delivery system. The Physician will perform administrative tasks as assigned such as peer review and Ongoing Professional Practice Evaluations. The physician will provide oncology on-call services as assigned.

Place of Performance:
Services shall be provided at the Government Facility site:
• Buffalo NY 14215 (Primary site);
• Batavia NY, 14020;
• Olean NY 14760

Travel:
There will be an expectation to conduct clinic at the Batavia and Olean in accordance with patient volume for oncology services. The frequency of clinic at these locations will be reoccurring with a minimum of one day per quarter and a maximum of one day per week for each location. Batavia is 36.4 miles from the primary location in Buffalo. Olean is 75.5 miles from the primary location in Buffalo. We estimate there may be a maximum of 26 visits per year to either the Batavia or the Olean.

Government-Furnished Information, Equipment, and Facilities.
Physicians will be given laptops to complete work. Physicians will be given necessary equipment to complete procedures and will have space at the Buffalo location to complete work. Scheduling of patient will be completed by staff. Monitoring of access and other important metrics will be conducted by Staff.

Qualifications:
License:
The physician(s) assigned by the Contractor to perform the services covered by this contract shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia when services are performed onsite on VA property. All licenses held by the key personnel working on this contract shall be full and unrestricted licenses. Contractor's physician(s) who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract.


Board Certification:
Contractor's physician(s) shall be Board Certified by the American Board of Internal Medicine Medical Oncology Board and be currently certified in Basic Life Support (BLS) Advanced Cardiac Life Support (ACLS) or equivalency. All continuing education courses required for maintaining certification must be kept up to date at all times.

Hours of Operation/Scheduling:
Physician(s) shall be available and present in clinic during normal facility clinic hours, which will be established, and may be revised, as deemed appropriate for patient care by the Chief of Staff. Currently, normal clinic hours are 8:00am to 4:30pm Monday through Friday excluding holidays.

Off-hours Coverage:
Physician(s) should be available on-call during all hours when the facility clinic is closed, including evenings, weekends and holidays.
a. On-call physician(s) must be available at all times for phone consultations with residents and physicians.
b. On-call providers must be available within 15 minutes by phone and on-site, within 60 minutes.
c. On-call schedule will be made in conjunction with staff oncology physicians

Standards of Care:
The physician(s) care shall cover the range of Oncology services as would be provided in a state-of-the-art health care facility and the standard of care shall be of a quality, meeting or exceeding currently recognized national standards as established by:
