Requirements:

• Board Certification – Oncology AND Hematology preferred
• CA State License – REQUIRED
• Medical Oncology Fellowship – REQUIRED
• CA DEA
• BLS
• ACLS – REQUIRED during credentialing
• Malpractice – Clean Preferred
• Must have FULL TIME availability – REQUIRED
