Requirements:

• CA State License – REQUIRED
• Medical Oncology Fellowship -REQUIRED
• CA DEA
• BLS
• ACLS – REQUIRED during credentialing
• Board Certification – Oncology AND Hematology preferred. Will consider Board Eligible.
