Requirements:

• Board Certified Occupational Medicine
• Active CO license or IMLC (LOQ in hand at time of submission)
• Active BLS
• Active DEA
• Level 2 Certified
• Candidates must detail the specific dates they are able to cover through May 31 2025
• Must acknowledge candidate understands the EMR training requirements
