Requirements:

• Board Certified – REQUIRED
• Active VA – REQUIRED
• BLS/ACLS Certified – REQUIRED
• DOT Certification – REQUIRED
• MUST BE WILLING TO COVER LOCATIONS AS LISTED IN JOB
• AVAILABILITY MUST BE NOTED AT TIME OF NAME CLEAR – Preference to providers that can cover ALL dates
• Immunization – Influenza – Current Flu Season; COVID; TB / QuantFERON TB Gold; MMR – REQUIRED TO NOTE CONFIRMATION AT TIME OF NAME CLEAR
