Requirements:

• Board Certification OB/GYN – Required or Truly BE (0-5 years of residency completion)
• Active UT License – Highly Preferred
• Active BLS or PALS Controlled substance – Required for credentialing
• Active DEA – Required for credentialing (site will consider a transferred DEA however must keep this DEA for the duration of the assignment not just for shifts worked.)
• Must be available for 1 week per month ongoing – Required
• Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date – Required
• Clean Malpractice/background – Highly Preferred – Must include self inquiry NPDB report at time of name clear.
