Requirements:

• Board Certified/Board Eligible – REQUIRED
• NE license or IMLC – REQUIRED
• ACGME/AOA accredited Residency Training Program in Obstetrics and Gynecology – REQUIRED
• ACLS
• BLS
• PALS
• DEA
• NRP – REQUIRED
• List availability at the time of name clear – REQUIRED
• Clean malpractice/background – HIGHLY PREFERRED
