Requirements:

• Board Certified (accepting truly BE within 5 years of training)
• KY license
• Fellowship Trained – Must complete a residency training program approved by the Accreditation Council for Graduate Medical Education or its equivalent in obstetrics at a minimum.
• Clean Malpractice
• Availability at time of name clear – REQUIRED
• Other Certifications – DEA and BLS/ACLS. Will need BLS but may be required to have higher certifications based on requested privileges. I.E – ACLS at a minimum if requesting Moderate Sedation.
