- Surgical GYN: Open, Laparoscopic Diagnostic/Minor only - Tubal & Chronal Ligation, LOA's
- Operative/Major (includes Diagnostic) - Ectopic Pregnancy, Ovary Removal, Cystectomy, Extensive LOA's
