Requirements:

• Board Certified OBGYN (No BE)
• MI License & CSR at Submission
• BLS
• Provider must be local
• Must have clean background for quick credentialing
• Submit rates at time of name clear
• Provide all availability if not available for listed dates
• DEA – Verbal Confirmation Required at Name Clear
