Requirements:

• Board Certified OBGYN – Required
• Active KS license – Highly Preferred
• Active BLS – Required for credentialing
• Active DEA – Required for credentialing
• Clean Malpractice/Background – HIGHLY PREFERRED
• Procedure Logs – REQUIRED BY BID DUE DATE. Providers without after the bid ends will NOT be presented.
