Physician – Neurologist – Locums

Requirements:

• Board Certified Neurological Surgery – Required or Truly BE (0-5 years of residency completion)
• Active CO License – Required or IMLC (LOQ in hand at time of submission)
• Active ACLS BLS – Required for credentialing
• Active DEA – Required for credentialing (site will consider a transferred DEA however must keep this DEA for the duration of the assignment not just for shifts worked.)
• Must disclose # of shifts per month provider can offer to the team ongoing – Required
• Candidates must be available for any dates detailed in the job description to be considered for ongoing dates – Required
• Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date – Required
• Clean Malpractice/Background (December shifts only) – Required
