Locums – Flagstaff, Arizona, Neurologist Job (Willing to License)

- Contract Length: ongoing
- Shift Details: Standard - Day shift - 5, 8Hr 08:00 - 17:00
- Schedule: Monday-Friday 8a-5p with 1 hour lunch
- Available Shifts: Oct 21,22,23,24,25
- Patients per day: 12 patients/day (36 patient-contact hours/week and 4 hrs. for charting, etc.)
- EMR: Cerner
- Hospital Bed Size: 317
- Case/Procedure Experience: BOTOX and ONB preferred – please list what they do and don’t
- Practice Setting: Hospital
- BC or BE in Neurology
- AZ License – Required or IMLC Accepted
- Willing to License
- DEA: Yes
- CSR: No
- ACLS – Preferred
- BLS - Required
- Credentialing Timeframe: 2 weeks
