We are seeking an experienced Nephrologist to join our patient-centered team. In this role, you will manage complex kidney disease cases, prioritize urgent referrals, and provide timely, comprehensive care both in-person and via telehealth (tele-nephrology).

Responsibilities include diagnosing and treating a range of nephrological conditions, educating patients on their disease process, and collaborating with tertiary facilities to ensure top-tier care. This position also offers a unique opportunity to extend care to underserved communities, focusing on bridging healthcare disparities through innovative technology and patient education.

Job Responsibilities

Key Responsibilities:
- Diagnose and manage a variety of nephrological conditions, including chronic kidney disease, - hypertension, and electrolyte disorders.
- Provide tele-nephrology services to improve access and continuity of care for patients across geographic barriers.
- Educate and empower patients to understand and manage their kidney health, with a focus on underserved populations.
- Collaborate with interdisciplinary teams and tertiary care facilities to ensure comprehensive care.
Participate in quality improvement initiatives to optimize patient outcomes and healthcare delivery.

Qualifications

Requirements:
- MD or DO with board certification in Nephrology
- Active medical license
- Experience or interest in telenephrology and working with underserved populations
