The Medical Director, Internal Medicine or Family Medicine Physician will lead a compassionate and innovative team for the House Call Division. This team delivers comprehensive, personalized medical care directly to patients in the comfort of their homes. Responsibilities
Provision of Primary Care
- Collaborate with an interdisciplinary primary healthcare team to deliver comprehensive primary medical care services to House Call patients, adhering to the highest standards of care.
- Conduct home visits to assess, diagnose, and treat patients in the comfort of their homes.
Patient Care Responsibilities
- Patient Assessments: Perform thorough evaluations of patients' medical history, physical condition, and medication usage during home visits.
- Diagnosis and Treatment: Diagnose medical conditions and develop individualized treatment plans, including prescribing medications as needed.
- Coordination of Care: Work closely with specialists, nurses, and other healthcare providers to ensure seamless continuity of care.
- Patient Education: Educate patients and their families about disease management, medication adherence, and lifestyle changes to promote better health outcomes.
- Emergency Response: Address urgent medical needs during home visits and coordinate referrals for specialized care when required.
- Documentation: Maintain accurate, detailed, and timely medical records, including patient assessments, treatments, and follow-up plans.
Operational Responsibilities
- Scheduling and Travel: Plan and manage a schedule of home visits, traveling to patient locations within the designated service area.
- Support Services: Facilitate access to additional patient services, such as physical therapy, diagnostic testing, or home health nursing.
Leadership and Administrative Duties:
- Team Management:
- Provide clinical guidance and mentorship to interdisciplinary team members.
- Oversee provider performance, conducting regular reviews, approving leave requests, and addressing HR-related matters.
- Deliver constructive feedback to providers and support their professional development.
- Monitor and review quality metrics with providers, ensuring adherence to standards of care.
- Coach providers on accurate coding, documentation, and compliance with regulations.
Collaboration and Mentorship
- Team Collaboration: Foster a collaborative environment by working closely with mid-level providers and care team members, such as community health workers.
- Teaching and Mentorship: Provide clinical training and guidance to team members, enhancing their skills and expertise.
- Medical Doctor (MD) or Doctor of Osteopathic (DO) degree from an accredited United States medical school
- Successful completion of residency in internal medicine or family medicine at an accredited healthcare institution
- Active NY License
- Board Certification or Eligibility
