As the Center Medical Director, you will:

  • Lead the clinical team to provide excellent patient care and foster a positive work environment.
  • Balance 80% clinical practice with 20% administrative duties.
  • Serve as a mentor and coach to providers, guiding them in delivering thoughtful, patient-centered care.
  • Participate in strategic initiatives to improve patient outcomes and support organizational goals.

This role allows you to continue practicing medicine while shaping the direction and success of the clinical team.

Job Responsibilities

1. Support of Organizational Strategy:

-Collaborate with the Practice Manager and Outreach Manager/Director to meet goals:
-Deliver best patient care.
-Ensure an unmatched patient experience.
-Drive exceptional growth.
-Promote a great place to work and leader in DE&I.
-Implement strategic initiatives in line with regional priorities.
-Communicate organizational information to the care team and escalate concerns or ideas.
Provider Management:
-Lead the onboarding of new providers.
-Model and ensure high standards of medical care and care model implementation.
-Support care teams in daily huddles, patient care planning, admission prevention, and
documentation workflows.
-Ensure care aligns with Oak Street’s clinical guidelines and keeps patients healthy and out of the
hospital.
-Educate providers on value-based, full-risk models of care.
-Address root causes when metric goals are not met.

Leadership and Coaching:
-Foster a culture of coaching and growth for providers and care teams.
-Provide performance reviews, set provider goals, and create plans for development.
-Address feedback from care teams constructively.

Culture:
-Celebrate successes and identify areas for improvement.
-Implement triad best practices.
-Support a positive, joyful culture within the practice.

Qualifications

M.D., D.O.
-Board certification in Internal Medicine or Family Medicine required for M.D. or D.O
-Experience in clinical leadership roles, leading and coaching providers to be the best they can be for their patients and their colleagues.
-Experience using a metrics-driven approach to the provision of medical care and/​or quality projects.
-Passionate about their work and motivated to be a part of a fast growing organization.
-Aware of their leadership skills and potential for impact and eager to share those with a fast-growing, energetic team on the leading edge of healthcare innovation.
-Commitment to older adults and to low-income patients.
-Comfort with ambiguity and a love for problem solving.
-Ability to remain calm in the midst of the storm, guiding others to smart, effective solutions.
-5+ years of experience in outpatient practice preferred.
-Fellowship training in Geriatrics, other professional degrees (e.g., M.B.A., J.D., M.P.H.), and prior executive roles welcomed but not required.
