Overview:
- Join the facility during a pivotal time for both the company and primary care.
- Lead in-home primary care provider teams in partnership with the Mobile Practice Manager.
Division Focus:
- Work within an interdisciplinary team.
- Provide primary care and transitional care support to high-need patients in their homes.
Key Responsibilities:
- Direct management, oversight, and coaching of the local in-home care team.
- Serve as part of the leadership team for the At Home division.
- Help develop and implement key programs, including:
- Complex care coordination
- Chronic disease management
- Psychosocial support
- Collaborate with:
- Clinical teams within the center
- Other Medical Directors
- Various internal and external partners
Job Responsibilities
Providing timely and constructive feedback to providers so the medical group continues to improve performance for patients and colleagues
Helping providers master the data-driven tools and practices required to keep patients well and out of the hospital, when avoidable
Reviewing regular quality dashboards with providers, coaching on tools to improve quality outcomes
Coach providers on appropriate coding and documentation
Provide managed care experience/teaching to providers
Additionally, the Medical Director will be part of the At Home leadership team that will have strategic input into a variety of divisional projects that support the enhancement of the care model, including:
Medical management, care management, utilization management, and quality improvement tools and processes
Development of clinical compliance policies and guidelines
Representation to external partners (e.g., vendors, payers, providers) and creating relationships and partnerships that support our mission and our value-based model
Additional responsibilities include:
Understanding and communicating provider performance/compensation plans and being able to answer questions and deliver results to direct reports
Serving as formal manager doing regular performance reviews and check-ins, approving paid-time-off requests, and managing miscellaneous human resource topics for providers in the market

Qualifications
Geriatric certification and/or other professional degrees (e.g., M.B.A, J.D., M.P.H) welcome but certainly not required
Experience with managed care and/or value-based practice
Experience using a metrics-driven approach to analyze cost, quality, and satisfaction data to drive clinical strategy and program redesign
An excitement for developing and implementing new processes
An understanding of the basics of managed care and a recognition that population health and a focus on the smart allocation of scarce resources can lead to high-value care (better care, lower cost)
An ability to remain calm in stressful situations
A commitment to older adults and to low-income patients
Comfort with ambiguity and passion for problem-solving
-Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate
-Internal Medicine or Family Medicine Board Certification (Or board eligible)
-Active, non-probationary, unrestricted State License
-Active DEA license
