Physician Affiliate Group of New York (PAGNY) is hiring a Director of Care Transitions at NYC Health + Hospitals/South Brooklyn Health. Accommodating more than 250,000 outpatient visits, and more than 15,000 inpatient admissions annually. Opportunity Details

  • Work with the Department of Medicine Chairperson and Director of Community Medicine to provide clinical direction to direct reports and community providers to set patient care standards, establish clinical goals and objects to ensure the strategic implementation of best practices and program development in their respective disciplines.
  • Manage care transition plans for inpatients referred by community providers
  • Ensure effective transparent communication with clinical and operational leadership regarding inpatient care transition plans within the division
  • Provide clinical administrative guidance and consultative direction to medical staff within the community medicine division, including oversight of patient documentation
  • Develop and collect performance metrics which the facility may utilize to plan, implement or administer quality patient care programs
  • Prepare division staff schedules for mid level provider coverage
  • Review, approve/deny and sign off on leave of absence requests, timesheets and evaluations for mid-level providers in the service
  • Provide direct care to patients assigned to the unit

Qualifications

  • A current, valid and unrestricted license to practice medicine in accordance with the laws of the State of New York
  • Board Certification in Internal Medicine
  • Minimum 5 years’ experience practicing in the medical specialty
  • Minimum 3 years’ supervisory experience, including demonstrated leadership and experience with personnel, finance, quality and clinical resource management
  • DEA registration
  • Status as a provider in good standing in the Medicare and Medicaid programs
