- BC in FM
- EMR is EHR NexGen
- two years of experience within a healthcare setting and leadership experience required
- Access health status of patient, review diagnostic reports, ensure the PC team nursing staff follows up with patients
- Develop and implement plan of care
- Perform minor surgical procedures
- Participate in peer review, quality assurance review, meetings, team hundles, particiapte in outreach and off-site health care programs in the community
- Reports to CMO
