Job Details:

• Employed outpatient opportunity
• Board-certified Internal Medicine, Family Medicine, or Geriatrics
• 50% clinical 50% admin
• clinic hours are 7:30 AM – 5 PM, M – F
• The SMD demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of at least 200-225 risk adjusted patients
• Act as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals, partner with the patient to work toward the goals, and frequently follow up on those goals on the path to improved health for their patients
• Manage day-to-day clinical and business operations to improve the clinical and financial performance of the center, as the true dyad partner to the Center Manager/Center General Manager
