We are currently recruiting for primary care staff physicians to join our team in Santa Rosa, CA. _x000D_

Active California License and good standing with the Medical Quality Control Board of the State of California._x000D_

Board Eligible or Board Certified in Internal Medicine._x000D_
Team Includes: On-site: Nurse Practitioner, Medical Assistant, Nurse, Operations Supervisor, Mental Health Support; Off-site: Professional team for Care Coordination and Referrals._x000D_

Job Summary: Under the direction of the Medical Director, or his or her designee, the Internal Medicine Physician will provide professional medical services as a member of the Patient Centered Medical Home (PCMH) Team._x000D_

Specific Tasks/Duties Include:_x000D_

Actively support organizational priorities of patient care, quality, education and sustainability._x000D_
Provide patient centered service and support a positive work environment._x000D_
Provide for the patients health care needs and when needed, arranges care with other qualified professionals._x000D_
Assure quality and safety by adhering to PCMH principles, including teamwork, evidence-based medicine, quality monitoring, and active participation of patients in decision-making._x000D_
Provide timely follow-up on patients seen including chart notations, lab review, and referrals to specialists including providing after hours coverage as required by site._x000D_
Participate in Team meetings, Quality meetings, and other professional activities as requested by the Medical Director._x000D_
Meets regularly with the Medical Director to review patient care issues and provides input into the development of treatment guidelines and protocols._x000D_
Provide on-site day to day clinical supervision and back-up of Family Nurse Practitioners and Physician Assistants. Provide back up for Family Physicians as needed._x000D_
Do chart review and give feedback to PCPs as directed by the medical director as part of ongoing quality assurance in geriatric care._x000D_
Participate in ongoing training._x000D_

Experience working with electronic medical records (EMR) preferred._x000D_
_x000D_
Knowledge and Skills:_x000D_
Demonstrated proficiency with all tasks on the most current Staff Physician competency list._x000D_
Complies with the clinical guidelines, policies and procedures._x000D_
Proficiency in Spanish (speak, read, and write) preferred._x000D_
Demonstrates knowledge of compliance issues within the community clinic environment._x000D_
Uses incident reporting system when appropriate._x000D_
Complies with HIPAA policies._x000D_
Completes assigned policy review items._x000D_
Strong interpersonal skills; ability to be sensitive with persons of various social, cultural, economic and educational backgrounds._x000D_
Experience with Outlook email function and EMR._x000D_
Ability to work as part of a team._x000D_
Good judgment, problem solving and decision-making skills.
