Schedule: Full-time (M-F)
Job Summary:
This role provides clinical primary care provided by physicians specifically trained for and skilled in comprehensive first contact and continuing care for persons with any undiagnosed sign, symptom, or health concern not limited by problem origin (biological, behavioural, or social), organ system, or diagnosis.
This position will work closely with the clinical team and is primarily responsible for managing direct patient care with the implementation of evidence-based medicine and adherence to national and internal practice guidelines and policies. Provides patient advocacy in the health care system to accomplish cost-effective care by coordination of health care services. Promotes effective communication with patients and encourages the role of the patient as a partner in health care.

- Annual Health Assessments (AHA)
- Chronic Care Management (CCM)
- Provides direct and indirect patient care as part of the treatment regime, identifying actual or potential risks based on patient screening data.
- Leverages the use of the Electronic Health Record ("EHR") to manage and improve patient care, workflows and clinical compliance.
- Provides patient advocacy in the health care system to accomplish cost-effective care by coordination of health care services.
- Promotes effective communication with patients and encourages the role of the patient as a partner in health care.
- Must be a board-certified in Internal or Family Medicine
- Successfully completed residency in their specialty
- Active and in good standing Medical Licensure in the state of California and Board Certified
- Active and good standing NPI number
- Active and good standing DEA number from US Drug Enforcement
