We are thrilled to share with you a rare and exciting opportunity to work with Prestige Outpatient Providers, LLC, an established post-acute practice currently expanding into the beautiful Sarasota County.

We are seeking a qualified physician who is looking to make a transition from hospitalist or primary care into the dynamic post-acute setting. This role is ideal for someone who enjoys setting their own schedule, takes full ownership of their work, is a team player, and is an excellent communicator and relationship builder. Most importantly, we are looking for someone truly passionate about the care they provide.

To begin with, this position will involve rounding in a large skilled nursing facility with a patient volume of around 100. With the right personality and motivation, this role has the potential to grow significantly. As the patient volumes increase, an advanced care provider will be available to assist with follow-ups and discharges. The earning potential in this role is uncapped.

Job Responsibilities

Comprehensive patient assessments
Development and implementation of personalized care plans
Collaboration with interdisciplinary healthcare teams
Monitoring and managing post-acute care transitions
Admissions need to be performed within 24-72 hours per CMS compliance regulation

Qualifications

Candidates must hold an active Florida Medical License. In addition, candidates must have an active
BLS, ACLS, DEA, as well as ability to provide med mal practice history from 4-10 years. We will also need: *American Board of Internal Medicine certificate or evidence of board eligibility
*An active driver's license
*3 professional peer references to verify clinical competency
*ECFMG, medical school diploma and residency certificate
*COVID or exemption
*Ability to pass the AHCA background screening

Working Hours

M-F: Hours typically are between 8a to 5p or when is communicated with the partnering facility.
