Your practice will be supported by integrated behavioral health clinicians, care managers, diabetic educators and a clinical pharmacist. Job Responsibilities

• Independently responsible for managing the health and well-being of patient panel.
• Responsible for assessment, diagnosis, development of treatment plan, interventions and evaluations.
• Comprehensive management of a broad range of health care, which includes promotion and maintenance of health.
• Management and coordination of care.
• Referrals, consultation and/or collaboration with other community healthcare providers and/or resources.
• Prescribing, dispensing and administration of therapeutic drugs, consistent within medical guidelines and scope of practice.

Qualifications

•    Unrestricted Oregon Medical Licensure and DEA 
•    Must Be Board Certified
