They currently boast a team of more than 20 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, transitional care, and remote patient monitoring.
POSITION DESCRIPTION:
The new physician will serve as the primary care provider for a dedicated panel of geriatric patients.
Patients are located primarily in facilities (ALFs / ILFs / SNFs), with a limited number residing in private homes. All patients are in the northern Kentucky area, south of Cincinnati.
In SNF settings (with only one location visited), physicians see an average of 15 patients per day.
If traveling between ALFs, ILFs, and/or private homes, physicians see an average of 9 to 12 patients per day.
Specific duties of the role include, but are not limited to:
- conduct health assessments to identify patient needs
- evaluate patient conditions and adjust treatment plans as necessary
- perform minor procedures and wound care as required (and as settings permit)
- educate patients and families on health management, disease prevention, and treatments
- work with other providers as needed to coordinate comprehensive care
The practice uses the eClinicalWorks EHR.
REQUIREMENTS:
- Kentucky medical license
- DEA
- Board Certified or Eligible, relevant field
- no experience required
