Healthcare United is actively seeking a Board-Certified Primary Care Physician (Internal Medicine or Family Medicine) to join our dynamic outpatient team in Pittsburgh, Pennsylvania. This role offers the opportunity to provide whole-person, team-based care in a collaborative and patient-centered environment.



Position Highlights

As a Primary Care Physician, you will:

  • Deliver comprehensive primary care services, with a focus on managing chronic conditions and preventing complications.
  • Work as part of an interdisciplinary care team including social workers, care managers, behavioral health specialists, nurses, pharmacists, and nutritionists.
  • Treat acute exacerbations of chronic conditions onsite when appropriate, ensuring continuity of care.
  • Offer same-day/next-day urgent appointments for patients recently discharged from hospitals, ERs, or skilled nursing facilities to prevent readmissions.
  • Develop strong transitional care plans to track patients throughout their healthcare journey, including hospital discharges and post-acute care settings.
  • Utilize evidence-based medicine and adhere to the Patient-Centered Medical Home (PCMH) model of care.
  • Optimize technology and Electronic Medical Records (EMR) to improve efficiency and quality initiatives.


Key Responsibilities

  • Diagnose, treat, and manage patients with multiple chronic conditions.
  • Educate patients on chronic disease management and healthy lifestyle choices.
  • Monitor and achieve quality metric goals to improve patient outcomes.
  • Attend multi-disciplinary team meetings to develop care plans and recommend treatment initiatives.
  • Provide on-call coverage for the office and hospital in rotation with other providers.

Requirements

  • Licensed Physician in the state of Pennsylvania.
  • Board Certification in Internal Medicine or Family Medicine.
  • BLS Certification required.
  • Experience in population health management is preferred.
  • Strong background in managing psychosocially and medically complex patients is highly desirable.
  • Familiarity with NCQA PCMH models preferred.
  • Proficiency in Electronic Medical Records (EMR) and Microsoft Office Suite.
  • Exceptional interpersonal and communication skills with a team-oriented approach.
  • Ability to adapt in a fast-paced, results-driven environment.
